How can I obtain a Death Claim form?
You may request a Death Claim form by calling our Customer Service Department toll-free at (800)628-5433. You may also e-mail us at info@baltlife.com, or click on the 'contact us' tab above.

When will I receive my check?
Your claim will usually be processed within seven (7) working days from the time it has been received in our corporate claims department. A lack of complete information is the biggest factor in slowing down payment. If there is additional information needed for processing not originally included with your claim, a request for the missing information will be made.

We need to use this policy to pay the funeral expenses. Can you pay the funeral home directly?
Yes. If you wish to have the proceeds paid directly to the funeral home, you must include an assignment of benefits form with your claim. This form can be obtained directly from the funeral home. It will need to be signed by each beneficiary who wishes to relinquish their portion of the benefits to pay the funeral expenses.

What information do I need to submit a claim?
A completed claim form, a certified death certificate, and the original policy(ies) are the basic requirements. There are some exceptions that will require additional documentation. For example, in the case of an accidental death, an accident report may be requested. If the beneficiary is listed as the estate, Letters of Administration showing the named executor of the estate will be necessary.

I don't know who the beneficiaries of the policy are. Will you tell me if I call?
Yes, if you are the owner of the policy.

What if the beneficiary is a minor?
Policy benefits cannot be paid directly to a minor. If no trust has been set up in advance for this purpose, benefits must be paid to the legal guardian of the minor child's property. Documentation designating the legal guardian will be requested with the claim.

We have a claim to file, but the beneficiary is also deceased. Who will get the benefits and who should file the claim?
There may be several possible answers to this question depending on the circumstances. In most cases, benefits would be payable to the contingent beneficiary listed in the policy. If there is no contingent beneficiary listed, it may be payable to the estate of the deceased or the estate of the deceased beneficiary, depending on who died first.
